Service Agreement & Waiver

Health & Safety

I confirm that my pet(s) are appropriately vaccinated and are free from contagious illnesses and active parasite infestations at the time of service. I will inform Meoki Paws of any known health conditions, injuries, allergies, behavioral concerns, or special needs that may affect my pet’s care or safety.

I understand that I am responsible for providing accurate and complete information about my pet(s) and their needs.

Emergency Veterinary Care

I authorize Meoki Paws to seek emergency veterinary care for my pet(s) if it is reasonably necessary. Meoki Paws will make reasonable efforts to contact me or my emergency contact before seeking veterinary care when circumstances allow. If I cannot be reached and immediate veterinary attention is reasonably necessary, I authorize Meoki Paws to seek veterinary care on my pet’s behalf.

I understand that I am responsible for all veterinary and related expenses incurred on behalf of my pet(s), including transportation costs if applicable.

Liability

I understand that caring for and handling animals involves inherent risks, including the possibility of injury, illness, escape, or unforeseen incidents.

To the extent permitted by law, I agree that Meoki Paws is not responsible for injury, illness, loss, or damage involving my pet(s) or my property arising from risks inherent to pet care or circumstances beyond the reasonable control of Meoki Paws, except where caused by negligence, gross negligence, or willful misconduct.

I accept responsibility for any damage or injury caused by my pet(s) to other persons, animals, or property while my pet(s) are receiving services from Meoki Paws, except where such damage or injury is caused by the negligence, gross negligence, or willful misconduct of Meoki Paws.

Home Access & Security

I grant Meoki Paws permission to access my home for the purpose of providing the agreed-upon pet care services. I will provide accurate and working keys, access codes, or entry instructions and will notify Meoki Paws of any changes to access information.

Meoki Paws is committed to treating your home and belongings with the same care and respect as we would our own. I understand that Meoki Paws will only access areas of my home that are reasonably necessary to provide the agreed-upon services and will make reasonable efforts to leave my home secure in the same manner in which it was found.

Meoki Paws agrees to keep all keys, access codes, and client information confidential and secure and will not share access information with others without my permission, except where required by law or necessary in an emergency.

Cancellations & Scheduling

I agree to provide at least 24 hours’ notice when cancelling or rescheduling a scheduled service. Cancellations or rescheduling requests made with less than 24 hours’ notice may be subject to a cancellation fee.

Meoki Paws reserves the right to modify, postpone, or cancel a scheduled service when weather conditions, emergencies, illness, or other circumstances create an unsafe or unreasonable situation for the pet or the service provider. When possible, Meoki Paws will communicate any changes as soon as reasonably possible.

Client Acknowledgement

By signing this agreement, I confirm that I have read and understood the terms outlined above and agree to the policies and conditions of service provided by Meoki Paws.

I confirm that the information I have provided about myself, my pet(s), and my home access is accurate and complete.

I understand that by checking the agreement box and submitting this form electronically, I am providing my electronic acknowledgment and agreement to these terms.
